Student Meal Account Form
To see the Student Meal Account Balance Options form, click here. This form will need to be printed and filled out, then turned in to the administration office or a campus office only if you are requesting a refund check or if you would like to transfer the money from one student to another. You do not need to fill out this form if you simply want your student's refund to go back into their own meal money account for later use.
Please be sure there is money in your child’s account. When sending checks, please be sure to put your child’s name on the check. If sending cash, please put it in an envelope with your child’s name. YOU MAY SEND ONE CHECK FOR ALL YOUR CHILDREN IF YOU CHOOSE. You may pay by the day, week, month, semester or year.
